Release and Waiver for Kid’s Heart Class Participation

As the parent or legal guardian of the minor participant named in the registration form, I understand that by registering for the Kid’s Heart Class, I am granting permission for my child to participate in this free, instructor-led art class.

I acknowledge and agree to the following:

1. Participation and Supervision

I understand that the Kid’s Heart Class is a creative, hands-on art activity that may involve the use of basic art materials and shared spaces. I agree that my child is participating voluntarily and that reasonable care will be taken by instructors and volunteers to provide a safe, positive environment.

I acknowledge that parents or guardians are responsible for determining whether the class is appropriate for their child’s age, abilities, and behavior.

2. Assumption of Risk

I understand that participation in any group activity carries inherent risks, including but not limited to minor injuries, spills, or accidents. I voluntarily assume all risks associated with my child’s participation in the Kid’s Heart Class.

3. Release of Liability

I release and hold harmless the Kid’s Heart Show, its organizers, instructors, volunteers, venue partners, and affiliates from any claims, demands, or liabilities arising from my child’s participation in the Kid’s Heart Class, except in cases of gross negligence or intentional misconduct.

4. Behavior and Conduct

I understand that respectful behavior is expected from all participants. The Kid’s Heart Show reserves the right to remove a participant from a class if their behavior is unsafe, disruptive, or harmful to others.

5. Photo and Media Release (Optional but Recommended)

I grant permission for photographs or videos of my child taken during the Kid’s Heart Class to be used for non-commercial, promotional, or educational purposes related to the Kid’s Heart Show, including use on websites, social media, and printed materials.

6. Medical Authorization

In the event of a minor injury or medical situation, I authorize the Kid’s Heart Show organizers to seek basic medical assistance for my child if I cannot be reached immediately. I understand that I am responsible for any related medical costs.

Acknowledgment

By submitting the registration form, I confirm that I am the parent or legal guardian of the minor participant(s) named and that I have read, understand, and agree to the terms outlined above.
